Mid-Course Survey | Template 1

This "Mid-Course Survey" is completely anonymous. To make this course most relevant and supportive of your learning, I would appreciate your completion. It does not show up in your grade book and there is no way for me to know who submitted what. The questions are open-ended and are designed so that you can find a way to submit feedback in a safe space. I want to improve as an instructor, and that requires feedback from students. This quiz is NOT for credit (I would have no way of knowing who took it, after all).
